  • Bridges to Change offers mental health and substance use disorder treatment. Our intake process is simple and we have a variety of programs designed to meet your specific needs. You can start the process today by going to www.bridgestochange.com/treatment

  • Substance Use Disorder Treatment: Intensive Outpatient Treatment is 9-18 hours a week. This includes groups, individual sessions with a licensed counselor, case management, consultation with a medical director and wellness activities.

    Medication Assisted Treatment (MAT): Recognizing the need for access to Medication Assisted Treatment for African Americans and Veterans, OCC has developed a culturally specific MAT program that allows access to a medical and clinical team that are experts in culturally specific practices. We combine medication dispensing with other therapies such as counseling, groups, case management and our wellness program.

    DUII Diversion and Conviction Program: Oregon Change Clinic (OCC) offers Oregon’s court-mandated Driving Under the Influence of Intoxicants (DUII) diversion program. Referral by an Alcohol and Drug Evaluation and Screening Specialist (ADES) is required for this program.

    OCC’s DUII diversion program is 14 weeks in duration.

    OCC’s conviction program is 16-20 weeks in duration.

    Oregon Change Clinic uses the evidence-based program “Responsible Decisions” for its curriculum.

  • We offer a Medication Assisted Treatment (MAT) Program for support with those that are seeking to decrease or abstain from substance use. Our program is harm reduction focused and emphasizes patient autonomy.

    The medications we offer range based on patient need. In general we offer the following:

    Opiate Use Disorder: Suboxone, Subutex, Vivitrol, Sublocade

    Alcohol Use Disorder: Naltrexone, Vivitrol, Campral, Disulfiram

    Stimulant Use Disorder: Naltrexone, Wellbutrin

    In addition to MAT, our patients engage in weekly to bi-weekly therapy for a minimum of 6 weeks to support long term recovery goals. Referrals to long term therapy are also available as needed.

    Group therapy is offered in person once a week on Tuesdays from 3-4:30pm

  • FSR Addiction Services: Finding and Sustaining Recovery (FSR) is an integrated behavioral health treatment program designed to help those dealing with addiction build sober and healthy lifestyles. FSR Addiction Services include Acupuncture, Chinese Medicine, Wellness Movement, Group Therapy, Peer Support, Sober Housing, Individual Therapy, and Nutrition.

  • Recovery Toolkit Series: This holistic, multi-disciplinary service model is built on evidence-based practices that enable people with substance use disorders to create individualized recovery toolkits according to their own needs. The Alano Club is the first organization of its kind in the United States to pioneer this comprehensive and innovative approach to recovery in a community-centered environment—all free of charge.

  • Our unique approach of using the most current medically assisted therapies along with intensive drug and alcohol counseling has been incredibly successful at achieving sustained recovery. We want you to have the greatest chance at recovery you possibly can. We work with every patient with their own individualized plan.

    Our team of physicians and medical providers are trained and experienced in Medication-Assisted Treatment for opioid and alcohol use disorders. While medication alone cannot result is sustained sobriety, it is a invaluable tool that helps support and expedite the recovery process.

  • Navigate Back Home is a trauma informed pilot program that provides 10-20 women opportunities to continue to work on their journey of recovery, reunite with their children, and rebuild their lives. Our Housing Referral Coordinator will ensure each woman receives support and guidance to obtain stability for long-term continuum care until they receive permanent housing. Further, the Housing Navigator will continue to connect with housing agencies, such as Home Forward, second chance landlords, homes for single women, and women with children to identify what housing opportunities are available.

  • Addiction Services: We at HHBH understand that African American clients who struggle with substance abuse and/or mental health disorders experiences, look very different than that of the men and women of the dominant culture, as it relates to their experience with the legal system, treatment encounters and accountability.

    DUII Services: HHBH DUII program ensures appropriate screening, education, and substance use disorder treatment services for individuals who are under a diversion agreement for, or convicted of, driving under the influence of intoxicants (DUII) in an Oregon court. The service is offered to aid in completing court requirements and to reduce the risk of re-offending.

    Gambling Services: HHBH gambling counseling is effective because its purpose is not just to stop the gambling, but to eradicate the DESIRE to gamble.
